I am aware of the fact that you have to purchase multiple licenses for each computer that you want to run FirstSpot on but if I were to buy the $395 standard version will I have to continue to pay $395 every year or is that just for maintenance? Essentially, is the $395 yearly to keep the license?

The license is permanent and there is no yearly fee. Only the maintenance (which includes official support and free upgrade) you need to pay yearly to renew (optional).
